Care Coordinator / Community Health Worker
The Care Coordinator / Community Health Worker helps patients and families access and participate in health care, nutrition, and community support services. The position combines care coordination with community outreach, culturally responsive education, advocacy, and practical assistance to address barriers affecting patient health and well-being.
Key responsibilities include:
- Conducting outreach to patients and families and helping them understand available services.
- Coordinating medical and nutrition appointments, referrals, transportation, and follow-up.
- Identifying barriers such as food insecurity, transportation difficulties, and challenges accessing benefits or community resources.
- Assisting with enrollment and participation in food and nutrition programs, including coordinating food pickup or delivery.
- Reinforcing health information provided by clinical staff and helping patients communicate their needs and concerns.
- Tracking referrals and patient participation, maintaining confidential records, and communicating unresolved needs to the care team.
Work takes place in clinic and community settings and may include home visits. This is a nonclinical position.
Qualifications
High school diploma or equivalent and experience in community outreach, health care, patient services, or a related field. Strong communication, computer, and organizational skills are essential.
Compensation
Salary Range: $55,000 – $65,000 annually, depending on experience, plus fringe benefits.

Family Practice/Urgent Care Physician (MD/DO)
Works in collaboration with other healthcare professionals to deliver patient/family-focused care. Provides the full range of primary medical care for patients of all ages, including urgent/emergent medical services. Call is required.

Medical or Osteopathic doctorate degree from an accredited medical school. Completion of residency in Family Medicine at an accredited US residency program. Licensed to practice medicine in the State of Hawaii; Board Certified/Board Eligible in Family Medicine by the American Board of Family Medicine; ACLS, PALS and BLS certification; Current DEA and Hawaii State Narcotics registration.
Compensation
Salary Range: $220,000 – $240,000 annual

Medical Assistant – Full Time
Hāna Health is seeking a dependable, compassionate, and organized Medical Assistant to support our primary care team. Working under the supervision of licensed medical providers and the Clinic Manager, you will help deliver patient-centered care and keep daily clinic operations running smoothly.
KEY RESPONSIBILITIES
- Record patient histories, vital signs, medications, allergies, and other information accurately in the electronic health record.
- Prepare patients and assist providers with examinations, procedures, and treatments within your training and assigned responsibilities.
- Collect and prepare laboratory specimens; assist with EKGs, dressings, and other clinical tasks as directed.
- Provide patient instructions and visit materials; help coordinate follow-up appointments, referrals, and care needs.
- Prepare, clean, disinfect, and stock exam rooms; sterilize instruments and maintain supplies and equipment.
- Assist with telephone calls, patient check-in and check-out, insurance verification, payment collection, and billing documentation.
- Protect patient confidentiality and follow established infection prevention and safety procedures.
Qualifications
- High school diploma or equivalent and current Basic Life Support (BLS/CPR) certification.
- Medical Assistant certification from an accredited program and previous medical office experience preferred.
- Strong communication, organization, and computer skills, including the ability to maintain accurate electronic health records.
- Ability to work independently and as part of a team, adapt to changing priorities, and treat patients and families with professionalism and compassion.
Compensation
$23–$28 per hour, depending on experience and certification.

Medical / Dental Receptionist
Hāna Health is seeking a friendly, dependable, and organized Medical/Dental Receptionist to welcome patients and support our medical and dental teams. Join us in providing caring, respectful service to our community.
KEY RESPONSIBILITIES
- Greet patients and assist with registration, forms, and check-in and check-out.
- Answer telephone calls, take messages, and schedule and confirm appointments.
- Verify insurance coverage and maintain accurate patient information in the electronic health record.
- Process billing information, collect payments, and issue receipts.
- Coordinate with medical, dental, and billing staff to support smooth patient flow.
- Protect patient confidentiality, maintain an organized reception area, and perform other clerical duties as assigned.
Qualifications
- Positive attitude and strong communication and customer service skills.
- Good organizational skills, accuracy, and attention to detail.
- Ability to manage multiple tasks and remain calm in a busy environment.

HĀNA FRESH
Farm Manager – Hāna Fresh
Located in the remote district of Hāna on the island of Maui, Hana Fresh is a 7 acre certified organic farm with an additional 30-acre parcel under development. The farm produces fresh market vegetables and fruits for distribution through the Hana Fresh farm stand and meal programs as well as retail outlets and restaurants.
The Farm Manager oversees and participates in the development and coordination of both field and greenhouse operations and assures facilities, irrigation and equipment are appropriately maintained. Responsible for managing inventory and the organic certification process. Coaches and trains farm labor from diverse backgrounds and differing skill sets in completion of daily tasks including seeding, planting, weeding, harvesting, washing, packing, and record keeping. Must be “production oriented” and a willing participant in all farming activities.
Qualifications
Agriculture Degree and experience farming in sub-tropical climate preferred. At least three years of experience working in organic farming systems and supervising staff. Works the hours needed to get the job done. Excellent verbal and written communication skills.
Compensation
Salary: $85,000 – $95,000 annual Benefits
