Rx for Health: Clinical Nutrition Support

Patients who are at risk of developing chronic health conditions receive a prescription for fresh produce at the Hāna Health farm stand. Transitioning to a diet rich in fruits and vegetables can help prevent the onset of diabetes, high blood pressure and other chronic conditions.

Rx Card for Fresh Produce

  • Use your issued Rx card to shop for fresh produce only at the Hāna Fresh farm stand on Tuesdays and Thursdays
  • $50 will be added to your Rx card each month after completing you monthly weigh-in and short survey.
  • Any unused balance will roll over to the next month
  • Please note: the Rx card cannot be used for prepared foods or other non-produce items.

Monthly Check-ins

  • Each month, visit the Hale to complete a quick weigh-in and short survey.
  • No appointment needed — simply let the receptionist know you’re there for your Rx
  • You’ll receive a reminder each month to help you stay on track.

Quarterly Dietician Consultations (or as frequent as desired)

  • Every three months, youʻll meet with our registered dietitian for personalized nutrition guidance and practical tips to support your wellness goals. (Quarterly is the minimum but if you are interested, you can meet with dieticians more frequently if desired.)
  • Increase your daily intake of fresh fruits and vegetables.
  • Apply the nutrition guidance provided by your dietician.
  • Experience improvements in energy, mood, sleep and weight
  • Make small, sustainable changes to support long-term health.
  • Track your progress through monthly check-ins and surveys.
