Administrative Assistant
Provides high-level administrative support for Executive Director. Clinic Director
Responsible for overall operation of the health center, including personnel management, facility management, and quality improvement activities. Coordinates patient care in a multidisciplinary setting.
Responsibilities
Provides management to clinic staff.
- Determines the most effective method for assigning responsibilities and duties to department employees
- Maintains job descriptions, procedures and other documentation related to the organization of the department
- Assigns duties and responsibilities, and ensures employees receive instruction/training needed to complete their job responsibilities
- Ensures that employees are aware of and adhere to all company policies and procedures, and conveys all senior management communications and directives
- Provides mentoring/guidance to assist in employee development
- Reviews departmental work for thoroughness and accuracy, and provides specific instructions to supervisors and employees on completion of tasks/responsibilities
- Prepares and conducts performance appraisals for immediate staff
- Conducts hiring, disciplinary, and termination procedures
Responsible for quality, clinic policy, and risk management including:
- Manages the Quality Improvement program
- Assists executive director with the development, monitoring, and enforcement of standards for infection control, emergency preparedness, risk management and OSHA requirements
- Monitors confidential information processing and assures clinic HIPPA compliance
- Audits patient records, lab and referral logs, and other tracking logs to assure documentation of patient care services
- Verifies emergency equipment availability, completes preventative maintenance requirements, troubleshoots malfunctions, calls for repairs, maintains equipment inventories and evaluates new equipment and techniques in order to ensure proper operation of all health center equipment, in collaboration with other management staff
- Monitors inventory and coordinates purchasing of supplies
- Participates in the development and use of the EHR
- Participates in the development of health centers policies and procedures
- Participates in the development of annual operating, quality improvement, and risk management plans
Qualifications
A Bachelor’s Degree and a minimum of 3 years of professional management experience is required.
Skills/Abilities
Displays significant organizational skills, in order to manage and direct the activities of the clinic. Possesses specific advanced skills, including written and verbal communications skills, computer and technical skills, and mathematical knowledge frequently acquired through completion of a Bachelor’s Degree program with a recognized major. Possesses an understanding of the profession at a level that allows for effective decision-making. Ability to resolves conflicts that may arise because of disagreements between employees, between employees and customers/clients, or with the public.

Community Health Worker

Family Practice / Urgent Care Physician (MD/DO)
Works in collaboration with other healthcare professionals to deliver patient/family-focused care. Provides the full range of primary medical care for patients of all ages, including urgent/emergent medical services. Call is required.
Requirements
Medical or Osteopathic doctorate degree from an accredited medical school. Completion of residency in Family Medicine at an accredited US residency program. Licensed to practice medicine in the State of Hawaii; Board Certified/Board Eligible in Family Medicine by the American Board of Family Medicine; ACLS, PALS and BLS certification; Current DEA and Hawaii State Narcotics registration. Must be credentialed with all major insurance plans. Supervises and mentors Physician Assistants and Nurse Practitioners. Serves as a role model for all staff by setting a good example, which includes but is not limited to punctuality, demeanor, communication, participation and customer service. Must be committed to a team approach and the Patient Centered Medical Home model of care. Proficient in the use of EHR. Charting is routinely to be completed at the end of the patient visit. Proper coding and appropriate charging for all services rendered is required.

Quality Improvement Manager
Responsible for coordinating all quality management efforts including but not limited to:
- Works with the Executive Director, medical staff and others to develop performance improvement targets for quality, service, and efficiency of the organization.
- Provides leadership for implementing changes targeted at systems improvement. Measures and evaluates attainment of results.
- Assists with the development of protocols designed to implement national standards of care.
- Development and implementation of the Quality Improvement Plan and annual work plan that establishes a systemic, organization-wide approach to meeting quality measures
- Active participation in “pay for performance” opportunities including those offered by health plans, PQRS and Meaningful Use.
- Achievement of PCMH certification.
- Identification of applicable licensure, accreditation, credentialing regulations and standards.
- Identification of staff training needs and development/implementation of training plans.
Education
Graduation from an accredited School of Nursing. Current Hawaii state license as a professional Registered Nurse required.
Experience

Registered Nurse
Actively participates in the delivery of patient care including vital signs, phlebotomy, laboratory, medication administration and under the auspices of the physician assists with medical procedures, emergency care, x-rays. Coordinates referrals and case management. Education

Experience
At least two years current experience as an RN. Experience/comfortable with the use of electronic health records. Emergency room experience desirable. Experience in a rural community-based health setting preferred.

Farm Manager
Hana Fresh is a small 6 acre certified organic farm located in the remote district of Hana on the island of Maui. The farm produces fresh market vegetables and fruits for distribution through the Hana Fresh farm stand as well as retail outlets and restaurants. The Farm Manager oversees the development and coordination of the farm operation from crop production to packing for sales. Responsible for crop management tasks including but not limited to: pest and disease management, irrigation and fertility management, compost and egg production. Other general farm tasks include seeding, planting, weeding, harvesting, washing, packing, and record keeping. This is a “hands on” position.

Prepares and serves beverages and food products in a quick and efficient manner. Operates the cash register. Responsible for providing excellent customer service and maintaining the highest standards for all product preparation. Maintenance of the farm stand area including all equipment and supplies.
